Who we are

This website and the telehealth consultations described on it are provided by Neal Shah Nephrology PC, a Maryland professional corporation ("the practice", "I", "me"). Contact: shahnephrologypc@gmail.com.

Information I collect

When you use the contact form, I collect your name, email address, where you will be located during the visit, and the short note you write. Please do not include medical details in the form.

If you become a patient, I collect the health information and records you share with me so that I can provide your consultation, along with billing details needed to send and process your invoice.

The website host may record basic technical information, such as your browser type and pages visited, as part of normal website operation.

How I use your information

To reply to your request, arrange and provide your consultation, send invoices and receipts, keep medical records as required by law, and meet legal and professional obligations.

Payments

Invoices are paid through a third party payment processor. Your card or bank details are entered directly with that processor and are not stored by the practice.

Sharing

I protect your information and do not sell it. I share it only with your permission (for example, with your other doctors), with service providers who help run the practice (such as the website host, email provider, video platform and payment processor), or when required by law.

Your choices
