About these terms

These terms apply to this website and to telehealth consultations provided by Neal Shah Nephrology PC, a Maryland professional corporation. By using this website or booking a consultation, you agree to these terms.

Who can receive care

Consultations are available only to patients who are physically located in Maryland or in India at the time of the visit. Your location is confirmed at the start of each visit. If you are not in one of these locations, the visit cannot go ahead.

Services

Consultations include new patient consultations, follow up visits, lab and record reviews, and second opinions, provided by video or phone. A doctor and patient relationship begins only after a consultation is confirmed and completed. Information on this website is general and is not medical advice.

Not for emergencies

Telehealth consultations are not for urgent or emergency care. In an emergency, call 911 in Maryland or 112 in India, or go to the nearest emergency department.

Fees and payment

Fees are discussed and confirmed in writing before the visit, then sent as an invoice. Payment is due before the consultation. Fees are in US dollars. Patients in India may see the invoice converted to rupees at checkout, and the payment processor may add a small currency conversion charge.

Insurance

I do not accept insurance. Fees are paid directly by the patient. A receipt is available on request, but I do not file insurance claims.

Cancellations and refunds

If you cancel before the visit starts, you receive a full refund. There is no refund after a completed visit. If I need to cancel, you receive a full refund or a new time.

Your responsibilities

Please give accurate information about your health, medicines and location, and keep your local doctors informed. Follow up with in person care when advised.

Limitation of liability

To the extent permitted by law, the practice is not liable for indirect or consequential losses arising from use of this website.

Governing law

These terms are governed by the laws of the State of Maryland.
